Licensed as a CA LVN and one (1) year clinical experience in a health related field. Strong written and verbal communication skills. Knowledge of medical terminology, and experience in evaluation, direct client service and community linkage. Ability to work independently and as a member of the multidisciplinary team and show mature judgement. Ability to collaborate with staff from other disciplines and with community agencies. Valid CA driver’s license and good driving record is required. Must be 21 or over to drive.
Health and Prevention Care Manager participates in multidisciplinary evaluation of each client’s physical and mental health needs. Participates in the development of individualized and integrated care coordination plans. Provides medication support, education and organization within scope of license.
