The Crisis Transition Teams (CTT) are developed under SB82 to augment the services of the Exodus Recovery Urgent Care Centers (UCC) by providing linkage and short-term mental health services to special needs populations who require additional supports upon leaving the UCCs. The Crisis Transition Teams are located in each UCC and consist of a Crisis Transition Coordinator and a Crisis Transition Case Manager who will work together to link persons with mental health issues into the appropriate services needed to achieve recovery. These teams will provide intensive case management for up to 60 days following discharge from the UCCs to ensure stabilization, linkage to on-going services/supports and triage services within the consumers’ local communities.
The Exodus Foundation for Recovery is a 501(c)(3) entity that has historically supported the mission of Exodus Recovery, Inc. (Exodus) in providing quality behavioral health services to disadvantaged and vulnerable individuals with mental illness, substance use disorders and chronic physical health issues. The Foundation was originally founded to subsidize housing services and provide treatment for individuals in need with limited resources. At MLK Medical Center the Exodus Foundation is pleased to partner with the LA County Department of Health Care Services (DHCS) and the Department of Mental Health (DMH) on the development of a mental health Urgent Care Center and an Integrated Clinic, specifically designed to serve the needs of the local community and collaborate with community providers and partners in building a strong system of care in Willowbrook and South Los Angeles.
