The Exodus Whole Person Wellness Project will provide comprehensive care coordination and service navigation to identified homeless individuals in the North San Diego County region. The Social Worker will serve as the primary service coordinator for program participants. Responsible for delivery of clinically related services such as assessment, individual rehabilitation services, crisis intervention and case management. Assess clients for safety and risk factors, develop treatment and discharge plans and facilitate groups. Responsible for linking clients to community resources, assists in the application for benefits and serves as a liaison to establish and maintain community partnerships. Operates in a culturally competent, ethically sound and professional manner.
Bachelor’s degree in social work, human services or related field required. Master’s degree preferred. Two years’ substance abuse and mental health clinical experience preferred. Experience working with homeless individuals desired. Strong knowledge of behavioral health assessment, therapeutic interventions, community resources and crisis intervention required. Possess the ability to work independently, set priorities and good time management skills. Bilingual preferred.
