Masters’ degree required. Two years substance abuse and mental health clinical experience preferred. Experience working with homeless individuals required. Strong knowledge of behavioral health assessment, therapeutic interventions, community resources and crisis intervention required. Possess ability to work independently, set priorities and good time management skills. Bilingual preferred.
The Social Worker will serve as the primary service coordinator for program participants. Responsible for delivery of clinically related services such as assessment, individual rehabilitation services, crisis intervention and case management. Assess clients for safety and risk factors, develops treatment and discharge plans and facilitates groups. Responsible for linking clients to community resources, assists in the application for benefits and serves as a liaison to establish and maintain community partnerships. Operates in a culturally competent, ethically sound and professional manner.
